What Personal Information is Being Collected?
The personal information that will be collected will include IP addresses, and other personal information that you volunteer, such as names and emails. If any additional information is collected in the future, consent will be sought prior to such collection if required.
Purposes for Collection
Data will be used to identify and improve emotional and mental wellness supports in the community.
Sharing the Results
Survey responses will be reported in aggregate to ensure participant confidentiality. The reports will be made publicly available on the Whistler Blackcomb Foundation website and social platforms, as well as to the Katz Amsterdam Foundation. The reports may be shared with health professionals, community partners, agencies, and the employment community as an important step to aid in the efforts of these groups to provide better emotional and mental health activities and support in Whistler.
Data Access and Storage
The Provider Survey is being delivered using Survey Monkey’s secure survey platform. Survey Monkey uses Amazon Cloud Storage servers in the United States or Amazon Web Services in Canada. Survey Monkey has a Privacy Policy and a Security Statement relating to the collection, use and security of its data. While using Survey Monkey to administer the Provider Survey, Whistler Blackcomb Foundation will remain fully in control of the Provider Survey data,
Survey data will be safely stored within Whistler Blackcomb Foundation’s secure, password-protected server and computer network. Access will be limited to the Whistler Blackcomb Foundation survey project team.
Risks and Benefits
We do not anticipate any harm will be caused to you by completing the Survey.
